Midstate EMS Regional Continued Quality Improvement — Original 2004, Updated 2018

Under the direction of the Midstate Regional Emergency Medical Advisory Committee (REMAC), the Midstate Region Continued Quality Improvement (CQI) Program develops, maintains, and enhances the quality of pre-hospital care within the Region.

Purpose

The goal of the CQI Program is to ensure the highest quality pre-hospital care possible in accordance with Public Health Law Article 30, Section 3006. Rather than asking "who caused this to happen?", CQI asks "what is wrong with the process and what can be done to improve the system?" CQI evaluates concerns, tracks indicators, and recognizes excellence.

Agency Level CQI

The primary level of the CQI program. Each agency is encouraged to maintain a CQI committee with at least three members, meeting no fewer than four times per year. Agenda items include emergent patient care concerns, PCR reviews, and continuing education. All records are kept confidential in a separate locked file.

Regional CQI Level

The secondary level, operating as a subcommittee of REMAC. Reviews overall system performance, agency CQI activity, and quality of care issues with potential regional effect. Identifies providers and agencies demonstrating excellence. Meets at least every other month or as needed for urgent issues.

Submit a Review Request

To request a call review, complete a Midstate Call Review Request Form and submit it to the EMS Clinical Coordinator via email, fax, or mail. Upon receipt, certified letters are sent to the agency and providers involved. The Regional CQI Committee will meet to review and notify all parties of the outcome.
